PART 1. VEHICLE AND FACILITY REQUIREMENTS  
R 456.1 Display of authorized vehicle decal.  
Rule 1. (1) A registered removal service shall place the department-provided decal of the  
authorized vehicle on the passenger-side interior of the windshield in the lower-right  
corner.  
(2) If a decal becomes damaged or unusable, a registrant shall notify the department  
immediately to receive a replacement.

R 456.2 Notice of change in location of refrigeration facility; new application  
required.  
Rule 2. (1) A licensed refrigeration facility shall immediately notify the department in  
writing of an intent to change the location of the facility.  
(2) A licensed refrigeration facility shall submit to the department an application and all  
fees for a new refrigeration facility location.  
(3) The department shall conduct an inspection of the new refrigeration facility when  
reviewing the application.  
(4) If an application for a new refrigeration facility location is approved, the license of the  
old location shall be closed.

PART 2. CHAIN OF CUSTODY  
R 456.3 System of identification of a dead human body.  
Rule 3. (1) Before transporting a dead human body that is accepted by a registrant, the  
dead human body must be affixed with a band or tag with the name and any other  
identifying information available of the deceased, including, but not limited to, date of  
birth, date of death, and name of authorized person requesting service.

(2) A dead human body that a licensee accepts for refrigeration must be affixed with a  
band or tag with the name and any other identifying information available of the deceased  
including, but not limited to, date of birth, date of death, and name of authorized person  
requesting service.  
(3) A registered removal service or a licensed refrigeration facility shall create a chain-  
of-custody log detailing all of the following:  
(a) The initial request for service, including the date, time, name of individual and  
authorized person requesting service, and the names of the employees who agreed to  
provide the service.  
(b) The date, time, and location when a dead human body was received into the care of a  
licensee or registrant, and the names of employees who received it.  
(c) General descriptions of all items that accompanied a dead human body while in the  
care of the licensee or registrant, including, but not limited to, any jewelry or clothing.  
(d) The date, time, location, and names of employees when a dead human body left the  
care of the licensee or registrant, and the name of the individual to whom the dead human  
body was released.  
(e) The name and any available identifying information of the deceased listed on the band  
or tag affixed to the dead human body as outlined in subrules (1) and (2) of this rule.  
(4) The registrant shall maintain a chain-of-custody log for not less than 7 years.
